Serum ferritin reflects the level of iron stored in your body, and according to the NIH Office of Dietary Supplements, it is one of the earliest and most sensitive markers of iron status. Ferritin starts falling in the earliest stage of iron depletion, well before hemoglobin budges. This is the whole reason the two results can disagree: ferritin is an early warning light, and hemoglobin is the engine finally sputtering.
The stages, in order
Iron loss tends to move through predictable phases:
- Stage one, storage depletion. Ferritin falls as your body spends down its reserves. Hemoglobin is still normal. You may already feel symptoms. This is low ferritin with normal hemoglobin.
- Stage two, iron-deficient production. Stores are gone and the body starts struggling to make red cells, though hemoglobin may still read within range. Other markers like transferrin saturation begin to shift.
- Stage three, iron deficiency anemia. Hemoglobin finally drops below normal. Only now does the standard CBC flag a problem.
The important takeaway is that a normal CBC rules out stage three. It does not rule out stages one and two, and those stages can make you feel unwell for months or years.
How you can have low ferritin with normal hemoglobin
Your body is protective of the oxygen-carrying job above almost everything else. When iron intake cannot keep pace with iron loss, your body does not immediately sacrifice your red blood cells. Instead it quietly raids the storage reserves first, pulling iron out of ferritin to keep hemoglobin production going.
