Iron Deficiency in Vegetarians: What the Science Says

Vegetarians and vegans are not necessarily more likely to be anemic, but they do tend to carry lower iron stores. A systematic review and meta-analysis of vegetarian diets and iron status found lower ferritin levels in vegetarians compared with people who eat meat. Ferritin is the storage form of iron, so lower stores means less buffer: a heavy period, a pregnancy, or a few months of poor eating draws the tank down faster.

The reason is not that plant foods lack iron. Many are rich in it. The reason is that plant iron is a different chemical form that the gut absorbs far less efficiently, and that several ordinary components of a plant-based diet actively block its uptake. Tea with a meal, wholegrains, legumes, and calcium-rich foods all reduce how much iron gets through.

The practical upshot is that a plant-based eater has more control than she might think. Absorption from a given meal can be varied several-fold by what is on the plate alongside the iron, and by what is deliberately kept off it. This guide covers what actually changes absorption, what a reasonable target intake looks like, and when a blood test matters more than a dietary tweak.

Heme versus non-heme iron

Dietary iron comes in two forms, and they behave very differently.

Heme iron is bound inside the haemoglobin and myoglobin of animal tissue. It is found in red meat, poultry, and fish. It is absorbed through a dedicated pathway that is relatively insensitive to the rest of the meal.

Non-heme iron is the inorganic form found in plants, eggs, dairy, and fortified foods. It is the only form a vegan diet supplies. Its absorption is highly variable and heavily influenced by everything else eaten at the same time.

A review of iron bioavailability and dietary reference values in the American Journal of Clinical Nutrition sets out the consequence: because non-heme iron absorption swings so widely with diet composition, the intake required to meet needs depends enormously on the overall dietary pattern rather than on the iron figure alone. Some authorities set a substantially higher recommended intake for vegetarians for exactly this reason.

This is why reading an iron figure off a nutrition label tells a plant-based eater much less than she expects. Two meals with identical iron content can deliver very different amounts into the bloodstream.

What blocks absorption

Four common inhibitors matter, and all four are staples of a healthy plant-based diet. That is the awkward part: the foods reducing absorption are not junk, they are the foods a careful eater eats more of.

Phytates

Phytic acid is found in wholegrains, legumes, nuts, and seeds. It binds non-heme iron in the gut and prevents uptake, and it is the single most significant inhibitor in most plant-based diets. It is also dose-dependent: even relatively modest amounts reduce absorption measurably.

Phytate content can be lowered by ordinary kitchen practice. Soaking legumes and discarding the water, sprouting, leavening bread with yeast or sourdough rather than eating unleavened flatbread, and fermenting all reduce it.

Polyphenols in tea and coffee

This one catches people out because the habit is so ingrained. A review in Critical Reviews in Food Science and Nutrition covering tea and other dietary factors found tea markedly reduces iron absorption. Coffee, cocoa, and red wine have similar though generally weaker effects.

The fix costs nothing: move tea and coffee out of the meal window. An hour either side of eating is a reasonable rule, and it is one of the highest-value changes a tea-drinking vegetarian can make.

Calcium

Calcium interferes with iron absorption, whether from dairy or from a supplement. This matters most for people taking a calcium supplement, who should separate it from iron-containing meals and from any iron supplement.

Certain fibres and oxalates

Spinach is the classic misunderstanding. It is genuinely high in iron and also high in oxalate, which binds it, so the iron delivered is far less than the label suggests. Spinach is worth eating. It is not a reliable iron strategy.

What helps absorption

Vitamin C, which is the big one

Ascorbic acid is the most powerful enhancer of non-heme iron absorption available, and the interaction has been studied for decades. It works by reducing ferric iron to the more absorbable ferrous form and by keeping it soluble in the small intestine, which also counteracts phytate.

Cheap ways to apply it: lemon juice over lentils or greens, tomatoes cooked with beans, peppers in a stir fry, a citrus fruit or kiwi with breakfast cereal, salsa with a bean dish.

Other enhancers

  • Fermentation and sprouting, which cut phytate as described above.
  • Cooking in cast iron, which transfers a small amount of iron into food, more so with acidic dishes.
  • Separating inhibitors from iron-rich meals rather than eliminating them. Tea at 4pm instead of with lunch changes the outcome without changing the diet.

Best plant iron sources

Worth prioritising, ideally paired with a vitamin C source:

  • Legumes: lentils, chickpeas, white beans, kidney beans, soybeans and tofu
  • Seeds: pumpkin seeds, hemp, sesame and tahini
  • Nuts: cashews, almonds
  • Wholegrains: quinoa, amaranth, oats, fortified cereals
  • Dried fruit: apricots, prunes, raisins
  • Dark leafy greens: kale, chard and broccoli, which are lower in oxalate than spinach
  • Blackstrap molasses and dark chocolate in small amounts

Tofu and tempeh deserve a specific mention: soy is high in iron, and fermentation in tempeh reduces phytate, which makes it one of the better-absorbed plant sources.

When diet is not enough

Dietary optimisation has a ceiling. It works well for maintaining adequate stores. It is slow and often insufficient for rebuilding stores that are already depleted, because the deficit can be substantial and absorption from food is limited per meal.

Signs that this has moved beyond a food question:

  • Persistent fatigue that rest does not resolve
  • Breathlessness on ordinary exertion, such as stairs
  • Unusual hair shedding
  • Brittle nails, or cracks at the corners of the mouth
  • Restless legs at night
  • Cravings for ice or other non-food substances, which is a specific and often-missed sign
  • Poor exercise tolerance or unusually slow recovery
  • Reduced concentration

Ask for a ferritin test, not just a full blood count. This is the single most useful thing in this article. Haemoglobin falls late in iron depletion, so a normal full blood count can sit alongside substantially depleted stores. Ferritin measures the stores themselves. Vegetarians and vegans should also have B12 status considered, since deficiency there produces overlapping fatigue symptoms and is more common on plant-based diets.

If you supplement

A few practical points that apply regardless of form:

  • Do not self-prescribe high-dose iron without testing. Iron overload is harmful, and some people carry haemochromatosis without knowing it. Test, then treat.
  • Take it away from tea, coffee, calcium and wholegrain meals. The same inhibitors that affect food iron affect supplemental iron.
  • Gut side effects are the usual reason people stop. Conventional oral iron salts deliver a large dose to the gut, and nausea and constipation are common. An iron supplement that is not taken does nothing, so tolerability is not a minor consideration.
  • Keep iron away from children. Accidental iron overdose is a serious poisoning risk in young children.
  • Rebuilding stores takes months, not weeks. Retest rather than guessing.

Do vegetarians need more iron than meat eaters?

The more useful framing is that the requirement depends on the whole dietary pattern rather than on a single number: how much vitamin C accompanies the iron, and how much phytate, tea, coffee and calcium are competing with it.

What is the best way to absorb more iron from plants?

Taking high-dose iron without knowing your status is not harmless: iron overload causes damage, and some people have undiagnosed haemochromatosis. Ask about B12 at the same time.

How long does it take to rebuild iron stores?

Months rather than weeks. Haemoglobin usually responds before ferritin, so feeling better does not mean the stores are refilled, and stopping early is a common reason deficiency returns. Retest rather than guessing, and if you have been supplementing consistently without any change, see still tired on iron and talk to your doctor, because absorption problems and other causes of fatigue both exist.
