The tablet-versus-capsule question matters far less than most people expect, because the shell is not what determines whether iron works or upsets your stomach. What actually decides both is the iron salt inside, the elemental dose, and how often you take it. A ferrous sulfate tablet and a ferrous sulfate capsule at the same dose behave much the same way.
Where format does matter is at the edges. Enteric coatings and slow-release designs genuinely reduce stomach irritation, but they do it by releasing iron further down the gut, past the duodenum where most absorption happens, which can reduce how much you actually absorb. Liquids let you fine-tune the dose. Dissolving films help people who cannot swallow tablets. Those are real differences, and none of them is "capsule versus tablet."
So if a supplement is not working or not tolerated, the productive questions are which salt, how much elemental iron, and how often. Swapping a tablet for a capsule of the same thing is the least useful change you can make.
What is actually different
Tablets are compressed powder, often with a film coating. They can be scored and split, they are usually the cheapest form, and they can be made in coated or slow-release versions.
Capsules hold powder or granules in a gelatin or plant-based shell. They dissolve in the stomach, sometimes slightly faster than a compressed tablet. They cannot usually be split. Vegetarians and vegans should check whether the shell is gelatin.
That is broadly the extent of it for absorption purposes. Both deliver their contents into the stomach and both rely on the same intestinal uptake afterwards.
Liquids and drops allow small, adjustable doses, which is genuinely useful for people who cannot tolerate a standard dose. They can stain teeth, which a straw mostly solves.
Orodispersible films dissolve in the mouth without water. A crossover study of an orodispersible film for iron supplementation in nine healthy women found the rate and extent of elemental iron absorption comparable to a well-regarded iron capsule. Worth noting honestly: small study, industry-funded, and it demonstrates that the format delivers iron, not that it takes a different route into the body.
The variable that actually matters: the salt

Ferrous sulfate is the standard and the most-studied. It is cheap, effective and the reference point in most trials. It is also the form behind most of the side-effect literature: a systematic review and meta-analysis in PLoS One found ferrous sulfate causes significant gastrointestinal side effects in adults.
Ferrous fumarate and ferrous gluconate are other common salts. Gluconate carries less elemental iron per milligram, so a "lower dose" is sometimes just less iron rather than a gentler form.
Ferrous bisglycinate is an amino acid chelate, frequently marketed as gentler. A review in Nutrition Reviews examined its effects on haemoglobin and ferritin concentrations in adults.

Read the elemental iron, not the compound weight
This is the most common label mistake. "Ferrous sulfate 325 mg" is the weight of the whole compound; the elemental iron it provides is a fraction of that. Different salts carry different proportions, so two products with identical-looking numbers can deliver very different amounts of usable iron.
Always compare the elemental iron figure. If a label does not state it clearly, that is informative in itself.
Enteric coating and slow release: the honest trade-off
These are marketed as gentle, and they often are. The mechanism is that they delay release until past the stomach, which reduces irritation.
The problem is anatomical. Most iron is absorbed in the duodenum and upper jejunum, near the start of the small intestine. A formulation designed to release later can deliver its iron past the best absorption site. Research on fractional iron absorption from enteric-coated ferrous sulphate has examined exactly this question.
The trade-off, plainly: better tolerated, potentially less absorbed. Whether that is a good deal depends on whether you would otherwise abandon the supplement, because an iron tablet that gets thrown in a drawer absorbs nothing at all.
What to change when iron is not working
In rough order of usefulness:
- Check the elemental dose. Very high doses raise side effects more than absorption. More is not faster.
- Change the frequency, not the shell. Iron raises hepcidin, which suppresses absorption of the next dose for about a day, so alternate-day dosing can absorb better than daily and often causes fewer side effects. The evidence is in how long it takes to raise ferritin. Discuss any change with your doctor rather than restructuring a prescribed regimen yourself.
- Fix what you take it with. Tea, coffee, calcium, dairy and wholegrain meals reduce absorption; vitamin C in the same sitting increases it. See why your iron may not be absorbing.
- Then consider the salt, if side effects rather than absorption are the obstacle.
- Then consider the format, if swallowing or timing is the practical barrier.
If you have taken iron consistently for three months with no change in ferritin, that is a finding for your doctor rather than a reason to buy a different brand. Ongoing blood loss, malabsorption and coeliac disease all present this way. See still tired on iron.

- Elemental iron stated clearly? If not, be sceptical.
- Which salt? This drives tolerability more than the shell does.
- Can you split it if you need a smaller dose? Tablets usually yes, capsules usually no.
- Gelatin or plant-based shell, if that matters to you.
- Enteric or slow-release? Understand the trade-off before choosing it.
- Does it fit your routine? Away from tea, coffee and calcium, on a schedule you will actually keep.
- Can you swallow it comfortably? A real constraint for more people than the category acknowledges.

One crossover study in nine healthy women found an orodispersible iron film gave absorption comparable to a well-regarded iron capsule. That is genuine but limited evidence: a small, industry-funded study. It shows the format can deliver iron effectively. It does not show that iron takes a different route into the body compared with swallowing a capsule.
Should I switch brands if my iron is not working?
Usually not first. Check the elemental dose, the timing relative to tea, coffee, calcium and dairy, whether you are taking vitamin C alongside, and how often you dose, since alternate-day dosing can absorb better than daily. If you have taken iron consistently for three months with no change in ferritin, that is a finding to take to your doctor. Ongoing blood loss, malabsorption and coeliac disease all look like a supplement that is not working.
Can I split an iron tablet to reduce side effects?
Often yes for plain tablets, especially scored ones, and it is a reasonable way to trial a lower dose. Do not split enteric-coated or slow-release tablets, because that destroys the coating that makes them what they are. Capsules generally should not be opened unless the manufacturer says so. Check with a pharmacist, and mention it to whoever prescribed it if the iron was prescribed.
